A preliminary study on the impact of pushrim-activated power-assist wheelchairs among individuals with tetraplegia

OBJECTIVES: This preliminary study was to evaluate the impact of pushrim- activated
power wheelchairs (PAPAWs) on mobility, community participation, satisfaction, and psychosocial impact among individuals with tetraplegia. DESIGN: Fifteen manual wheelchair users with tetraplegia completed a 4-wk protocol including a 2-wk trial where they used their personal wheelchairs for mobility and a 2-wk trial where they were provided with a PAPAW and could choose either wheelchair for mobility. The mobility levels with both wheelchairs were recorded by a datalogger. Participants completed daily questionnaires regarding community participation and medical scooters their satisfaction with the wheelchair used. The Psychosocial Impact of Assistive Devices Scale was used to compare the psychosocial impact of PAPAWs with their personal wheelchairs. RESULTS: Participants chose to use PAPAWs and their personal wheelchairs at a similar frequency in the PAPAW trial. The distances traveled with both wheelchairs were not statistically different (P = 0.09). Participants traveled significantly faster with PAPAWs (P = 0.03). PAPAWs did not result in significantly greater community participation, satisfaction, and psychosocial impacts than the personal wheelchairs. CONCLUSIONS: PAPAWs could be a viable mobility option for individuals with tetraplegia, which can provide independent mobility especially for outdoor activities. A user’s preference, life style, physical conditions, transportation issues, and environmental factors should be considered in prescribing such a device.

Mechanical efficiency and wheelchair performance during and after spinal cord injury rehabilitation

Admin January 21st, 2010

Mechanical efficiency and wheelchair performance during and after spinal cord injury rehabilitation

The purpose of the present study was to investigate whether mechanical efficiency (ME) relates to medical scooters propulsion capacity and wheelchair performance tasks during and after rehabilitation of people with a spinal cord injury (SCI). Eighty participants with a SCI were tested during rehabilitation (3 x) and 1 year after discharge. Two 3-minute submaximal exercise blocks, a maximal medicare power wheelchairs exercise test, and four wheelchair performance tasks were performed. ME, peak power output (PO (peak)), the sum of the performance times of a 15-m sprint and figure-of-eight, and the heart rate reserve (%HRR) during 10 s of wheelchair propulsion on a 3 % and 6 % slope were calculated. The relationship between ME and PO (peak), %HRR and performance time was tested with a multilevel regression analysis. ME showed a significant relationship with PO (peak) (p

Mechanisms of blood pressure and heart rate variability: an insight from low-level paraplegia.

Admin January 15th, 2010

Mechanisms of blood pressure and heart rate variability: an insight from low-level paraplegia.

It is still unclear whether the low-frequency oscillation in heart rate is generated by an endogenous neural oscillator or by a baroreflex resonance. Our aim was to investigate this issue by analyzing blood pressure and heart rate variability and the baroreflex function in paraplegic subjects with spinal cord injury below the fourth thoracic vertebra. These subjects were selected because they represent a model of intact central neural drive to the heart, with a partially impaired autonomic control of the vessels. In our study, arterial blood pressure and ECG were recorded in 33 able-bodied controls and in 33 subjects with spinal cord lesions between the fifth thoracic and the fourth lumbar vertebra 1) during supine rest (lowest sympathetic activation), 2) sitting on a power wheelchairs and handicap scooters (light sympathetic activation), and 3) during exercise (moderate sympathetic activation). Blood pressure and heart rate spectra, coherence, and baroreflex function (sequence technique) were estimated in each condition. Compared with controls, paraplegic subjects showed a reduction of the low-frequency power of blood pressure and heart rate, and, unlike controls, a 0.1-Hz peak did not appear in their spectra. Sympathetic activation increased the 0.1-Hz peak of blood pressure and heart rate and the coherence at 0.1 Hz in controls only. Paraplegic subjects also had significantly lower baroreflex effectiveness and greater blood pressure variability. In conclusion, the disappearance of the 10-s oscillation of heart rate and blood pressure in subjects with spinal cord lesion supports the hypothesis of the baroreflex nature of this phenomenon.

Perceptions of power mobility use and safety within residential facilities.

Admin January 9th, 2010

Perceptions of power mobility use and safety within residential facilities.

BACKGROUND: medicare power wheelchairs enhance quality of life by enabling occupation, improving self-esteem and facilitating social interaction. Despite these benefits, the risks associated with power mobility use raise serious concerns in residential facilities. PURPOSE: As there is no gold standard to assess when a client is unsafe, a two-phase study was conducted to develop client-centred guidelines for handicap scooters use. METHOD: In the first phase of the study, presented here, 18 in-depth, qualitative interviews were conducted with a variety of stakeholders, including power mobility users, other residents, staff and family members. RESULTS: A thematic analysis of the interviews revealed four main themes: 1) the meaning of power mobility, 2) learning the rules of the road, 3) red flags: concerns about safety, and 4) solutions. PRACTICE IMPLICATIONS: Given the importance of power mobility, safety measures need to address issues of mobility and safety for power mobility drivers and those around them.

How You Can Lose 20 Pounds On A Rapid Weight Loss Program

Admin January 8th, 2010

How You Can Lose 20 Pounds On A Rapid Weight Loss Program

Season Eight’s fifteen remaining contestants were confronted with an extraordinary second week challenge: lighten the collective weight by nearly ten pounds each in seven days. This contest required a massive team effort to hold each other responsible for reaching the goal. Each person on that team wanted to lose 20 pounds at least. This brought out an important aspect of any rapid weight loss program: answering to someone else for the results you get makes you work harder to get good results.

Failing on your own can seem natural, besides, you don’t have to tell anybody that you missed your goals.

The effects of arm crank strategy on physiological responses and mechanical efficiency during submaximal exercise

Admin January 2nd, 2010

The effects of arm crank strategy on physiological responses and mechanical efficiency during submaximal exercise

The purpose of this study was to compare submaximal physiological responses and indices of mechanical efficiency between asynchronous and synchronous arm ergometry. Thirteen medicare power wheelchairs -dependent trained athletes performed eight steady-state incremental bouts of exercise (0 to 140 W), each lasting 4 min, using synchronous and asynchronous arm-cranking strategies. Physiological measures included oxygen uptake (VO2), heart rate, and blood lactate concentration. The power wheelchairs outputs corresponding to fixed whole blood lactate concentrations of 2.0 to 4.0 mmol x l(-1) were calculated using linear interpolation. Mechanical efficiency indices - gross efficiency, net efficiency, and work efficiency - were also calculated. An analysis of variance with repeated measures was applied to determine the effect of crank mode on the physiological parameters. Oxygen uptake was on average 10% lower (P < 0.01), and both net efficiency (P < 0.01) and gross efficiency (P < 0.01) were higher, during the asynchronous strategy at both 60 and 80 W (gross efficiency: 16.9 +/- 2.0% vs. 14.7 +/- 2.4% and 17.5 +/- 1.8% vs. 15.9 +/- 2.6% at 60 and 80 W respectively). There were no differences in heart rate, blood lactate concentration or power output at either of the blood lactate reference points between the asynchronous and synchronous strategies (P > 0.05). In conclusion, test specificity is an important consideration. If a synchronous strategy is to be adopted, it is likely to result in lower efficiency than an asynchronous strategy. The exercise testing scenario may help dictate which method is ultimately chosen

Characterization of power wheelchair use in the home and community

Admin December 26th, 2009

Characterization of power wheelchair use in the home and community

OBJECTIVE: To characterize the use of power wheelchairs and to determine if multiple measures of mobility and occupancy jointly provide a more comprehensive picture of wheelchair usage and daily activity in full-time power wheelchairs users than daily distance alone. DESIGN: Prospective observational study. SETTING: Subjects’ everyday mobility was measured in their homes and communities for 2 weeks, and prompted recall interviews were conducted by phone. PARTICIPANTS: A convenience sample (N=25) of nonambulatory, full-time power wheelchair users. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Wheelchair usage was logged electronically, and geolocation and interview data were used to isolate chair use to (1) in the home, (2) not in the home indoors, or (3) outdoors. Distance wheeled, time spent wheeling, number of bouts, time spent in the wheelchair, and the percentage of time in the wheelchair spent wheeling were measured to describe wheelchair use. RESULTS: The median wheelchair user spent 10.6 hours (range, 5.0-16.6h) in his/her wheelchair daily and wheeled 1.085 km (range, 0.238-10.585 km) over 58 minutes (range, 16-173 min) and 110 bouts (range, 36-282 bouts). Wheelchair use varied across subjects, within subjects from day to day, and between environments. handicap scooters bouts outdoors were longer and faster than those wheeled indoors. In a regression analysis, distance wheeled explained only 33% of the variation in the number of bouts and 75% in the time spent wheeling. CONCLUSIONS: Power wheelchair use varies widely both within and between users. Measuring distance, time, and number of bouts provides a clearer picture of mobility patterns than measuring distance alone, whereas occupancy helps to measure wheelchair function in daily activities

Effect of a pushrim-activated power-assist wheelchair on the functional capabilities of persons with tetraplegia

Admin December 10th, 2009

Effect of a pushrim-activated power-assist wheelchair on the functional capabilities of persons with tetraplegia

OBJECTIVES: To test the differences between a pushrim-activated medicare power wheelchairs and a traditional manual wheelchair while performing common driving activities and to assess their relative merits for people with tetraplegia. DESIGN: Repeated measures. SETTING: An activities of daily living (ADL) laboratory within a rehabilitation research center. PARTICIPANTS: Fifteen full-time manual wheelchair users with tetraplegia due to a spinal cord injury. INTERVENTIONS: Participants propelled both their own manual wheelchairs and a PAPAW 3 times over an ADL course. The order in which the 2 different wheelchairs were presented to the participants was randomized. MAIN OUTCOME MEASURES: Each participant’s heart rate was monitored throughout testing by a digital, wireless heart-rate monitor. Time to complete the course was recorded, and participants were surveyed with a visual analog scale after the first, third, fourth, and sixth trials to determine the ease of completing each obstacle and their ergonomic preferences between the 2 wheelchairs. Participants also were observed throughout the trials to determine how much assistance they needed to complete each obstacle course. RESULTS: After using a Bonferroni adjustment, 4 obstacles (carpet, dimple strips, up a ramp, up a curb cut) were rated as being significantly easier ( P <.001) to complete when using the PAPAW. Participants also showed a significant decrease in mean heart rate throughout all 3 trials ( P =.015, P =.001, P =.003, respectively) when using a PAPAW. The amount of assistance needed by participants, the responses to ergonomic questions, and the overall time to complete the ADL course did not differ significantly between the 2 power wheelchairs. CONCLUSIONS: For subjects with tetraplegia, PAPAWs have the potential to improve functional capabilities during certain ADLs, especially when propelling up ramps, over uneven surfaces, and over thick carpet.

Effect of wheelchair stroke pattern on mechanical efficiency

Admin December 3rd, 2009

Effect of wheelchair stroke pattern on mechanical efficiency

OBJECTIVE: To investigate the effect of different medicare power wheelchairs and medical scooters stroke patterns on efficiency and propulsion technique (force application and timing). DESIGN: Inexperienced, able-bodied subjects were randomly divided into two velocity groups (1.11 m/sec n = 13 and 1.39 m/sec n = 11). An external (medium) load was set at 0.23 N/kg. Subjects performed four 4-min exercise blocks on a wheelchair ergometer. The first block was performed with a freely chosen movement pattern of the hand. Thereafter, the pumping, semicircular, or single-looping over propulsion pattern were performed in a counterbalanced order. Gross mechanical efficiency and propulsion technique variables were measured with Oxycon Alpha and an instrumented wheelchair ergometer. RESULTS: A significant difference was found for mechanical efficiency, with pumping showing the highest efficiency and semicircular the lowest efficiency, regardless of velocity. Timing variables and negative power deflections before and after the push phase showed significant differences between the stroke patterns. CONCLUSIONS: Pumping is the energetically most efficient stroke pattern in contrast to the semicircular pattern in this subject group. Propulsion technique could not explain the difference in efficiency.

Evaluation of a stair-climbing power wheelchair in 25 people with tetraplegia.

Admin November 26th, 2009

Evaluation of a stair-climbing power wheelchair in 25 people with tetraplegia.

OBJECTIVE: To compare the performance of a power wheelchairs with stair-climbing capability (TopChair) and a conventional power wheelchair (Storm3). DESIGN: A single-center, open-label study. SETTING: A physical medicine and rehabilitation hospital. PARTICIPANTS: Patients (N=25) who required power wheelchairs because of severe impairments affecting the upper and lower limbs. INTERVENTIONS: Indoor and outdoor driving trials with both devices. Curb-clearing and stair-climbing with TopChair. MAIN OUTCOME MEASURES: Trial duration and Quebec User Evaluation of Satisfaction with Assistive Technology (QUEST) tool; number of failures during driving trials and ability to climb curbs and stairs. RESULTS: All 25 participants successfully completed the outdoor and medicare power wheelchairs indoor trials with both wheelchairs. Although differences in times to trial completion were statistically significant, they were less than 10%. QUEST scores were significantly better with the Storm3 than the TopChair for weight (P=.001), dimension (P=.006), and effectiveness (P=.04). Of the 25 participants, 23 cleared a 20-cm curb without help, and 20 climbed up and down 6 steps. Most participants felt these specific capabilities of the TopChair–for example, curb clearing and stair climbing-were easy to use (22/25 for curb, 21/25 for stairs) and helpful (24/25 and 23/25). A few participants felt insecure (4/25 and 6/25, respectively). CONCLUSIONS: The TopChair is a promising mobility device that enables stair and curb climbing and warrants further study.

Miracle mineral supplement is the new wonder treatment

miracle mineral supplement is the new wonder treatment

There is a powerful electrostatic potential that exists between the chlorine dioxide moleculeand a pathogen. When they encounter one another a change takes place and there is a reaction between them. In the process, the pathogen lets go of up to five up its electrons. This is a fatal blow to the pathogen and a win for the body. Once the electrons have been taken, the mms minerals molecule is used up as well. It is quite harmless even with its extra electrons. It then begins to break down itself, into a harmless salt byproduct. The pathogen has been done away with and all is well. It’s an amazing process that works consistently well.

Table Tennis

Admin November 21st, 2009

Table Tennis

Table Tennis , also known as Ping Pong is a sport that can be played indoors and outdoors as long as the designated table divided by a net is present and outdoor conditions are ideal to play. But generally, table tennis is an indoor sport. The basics of the game involve two or four players hitting a ball back and forth to each with the use of paddles. If you have seen a professional table tennis match you’ll be awed by the quick reflexes the players have.

A prototype power assist wheelchair that provides for obstacle detection and avoidance for those with visual impairments

Admin November 20th, 2009

A prototype power assist wheelchair that provides for obstacle detection and avoidance for those with visual impairments

BACKGROUND: Almost 10% of all individuals who are legally blind also have a mobility impairment. The majority of these individuals are dependent on others for mobility. The Smart Power Assistance Module (SPAM) for manual wheelchairs is being developed to provide independent mobility for this population. METHODS: A prototype of the SPAM has been developed using Yamaha JWII power assist hubs, sonar and infrared rangefinders, and a microprocessor. The prototype limits the user to moving straight forward, straight backward, or turning in place, and increases the resistance of the wheels based on the proximity of obstacles. The result is haptic feedback to the user regarding the environment surrounding the medicare power wheelchairs. RESULTS: The prototype has been evaluated with four blindfolded able-bodied users and one individual who is blind but not mobility impaired. For all individuals, the prototype reduced the number of collisions on a simple navigation task. CONCLUSION: The prototype demonstrates the feasibility of providing navigation assistance to manual wheelchair users and medical scooters, but several shortcomings of the system were identified to be addressed in a second generation prototype.

Special considerations in the selection of the certified rehabilitation supplier.

Admin November 13th, 2009

Special considerations in the selection of the certified rehabilitation supplier.

During the last four decades, there have been revolutionary advances in the design of medicare power wheelchairs, complimented by special seating systems. Some of these advances include battery-operated power mobility, ergonomic steering devices, and a wide variety of innovative devices that allow the user to achieve a unique array of positions. These positions include tilting of the seat, reclining the back of the sear, achieving a passive standing position, and, finally, the unique ability of the wheelchair to climb over steep curbs cuts and even up staircases. Because of the availability of such a wide variety of wheelchairs, the medical profession requires the development of many new, unique partnerships. First, the user of a wheelchair must purchase the wheelchair from a certified rehabilitation supplier (CRTS). Realizing the importance of purchasing a wheelchair from a CRTS, there are other special considerations that the purchaser must make in the selection of a CRTS. First, the CRTS must have a coordinated team of healthcare personnel, including physicians, occupational therapists, physiatrists, and other seating specialists who are able to complete a pre-authorization form that will be approved by the private insurance company as well as Medicare and Medicaid. The CRTS must have a president as well as a successor who is familiar with the powered wheelchair and handicap scooters products, especially the powered wheelchair that can allow the user to achieve a passive standing position. In this comprehensive report, we will illustrate how an inexperienced CRTS who is not familiar with powered wheelchairs that can achieve a passive standing position can design and construct a wheelchair product that can seriously injure the user when they achieve a passive standing position. In Portland, Oregon, Tri-Care Wheelchair Service, Inc., is an experienced supplier of pow notered wheelchairs that achieve a passive standing position. Its two CRTS carefully assist the purchaser in selecting and then registering the powered wheelchair. Working with a team of seating specialists, they prepare preauthorization forms that will be approved by the private insurance company as well as Medicare and Medicaid. They carefully make adjustments in the design of the wheelchair seating system so that the user is comfortable as well as safe in a passive standing position.

Effect of visual perception, visual function, cognition, and personality on power wheelchair use in adults

Admin November 7th, 2009

Effect of visual perception, visual function, cognition, and personality on power wheelchair use in adults

The purpose of this study was to determine to what extent visual perception, visual function, cognition, and personality traits affect medicare power wheelchairs use in adults. It also proposes to establish baseline information to help clinicians determine or predict power wheelchair driving performance and to develop service plans to address those driving skills that need improvement or compensation. Sixty-two adult power wheelchairs users were recruited. Standardized instruments were used to evaluate visual perceptual skills, visual function, cognitive skills, and personality traits. The results of these evaluations were then correlated with participants’ scores on a power wheelchair performance test. Strong correlations were found between power wheelchair driving performance and visual perception (p = .000), ocular motor function (p = .000 and p < or = .001), stereodepth perception (p < or = .001), and alertness to the environment (p < or = .001). No significant correlations were found between personality traits and power wheelchair driving performance. These results indicate that good visual perceptual skills, visual function, and various aspects of cognition are necessary for proficient power wheelchair use. These data will assist clinicians in identifying significant factors to consider when evaluating and training clients for power wheelchair use.

Real-time head movement system and embedded Linux implementation for the control of power wheelchairs

Admin October 30th, 2009

Real-time head movement system and embedded Linux implementation for the control of power wheelchairs

handicap scooters has become very important for our quality of life. A loss of mobility due to an injury is usually accompanied by a loss of self-confidence. For many individuals, independent mobility is an important aspect of self-esteem. Head movement is a natural form of pointing and can be used to directly replace the joystick whilst still allowing for similar control. Through the use of embedded LINUX and artificial intelligence, a hands-free head movement wheelchair controller has been designed and implemented successfully. This system provides for severely disabled users an effective medicare power wheelchairs control method with improved posture, ease of use and attractiveness.

How mms sodium chlorite kills pathogens

Admin October 25th, 2009

How mms sodium chlorite kills pathogens

The potent molecules in chlorine dioxide can move about the body without any problems until they encounter a pathogen. A reaction then takes place between the two as the exchange of electrons takes place. This reaction can only take place where the ph is less than 7. This is a very acidicenvironment. Interestingly enough this is exactly the same type of environment that many of ourmodern diseases and stressors to the body thrive in as well. Bacteria, toxins and viruses love an acidic environment and therefore a beneficial arrangement is available. A positive charge typically is acidic. It is naturally attracted to the chlorine dioxide molecule and is happy to donate its electrons to them. It doesn?t hesitate to allow itself to be annihilatedin this way. Healthy tissues and healthy bacteria have a ph of 7 or more and will not donate their electrons or participate in a reaction.
Therefore no reaction can take place between the healthy tissueand the molecule. Jim Humble is the man responsible for MMS

Kinematic analysis for determination of bioequivalence of a modified Hybrid III test dummy and a wheelchair user

Admin October 17th, 2009

Kinematic analysis for determination of bioequivalence of a modified Hybrid III test dummy and a wheelchair user

We investigated whether a modified 50th-percentile Hybrid III test dummy (HTD) (First Technology Safety Systems, Plymouth, MI) would have motion similar to a medicare power wheelchairs test pilot (TP) with T8 paraplegia. Test cases were seated in a Quickie P100 electrically powered wheelchair (Sunrise Medical, Inc., Phoenix, AZ) driven at three speeds (0.8, 1.4, and 2.0 m/s). Three braking conditions-joystick release, joystick full reverse, and emergency power-off-were used to stop the power wheelchairs . The subsequent upper-body motion was recorded for the creation of kinematic exposure profiles of the wheelchair riders. The maximum concentration (Cmax) and area under the trunk angular displacement, velocity, and acceleration curves (AUC0-Cmax) were calculated. Assessments of average, individual, and population bioequivalence were conducted after data were subjected to natural logarithmic transforms. Only the Cmax of the trunk angular acceleration of the HTD and TP was average bioequivalent (0.82-1.04). Both Cmax and AUC0-Cmax measures for all kinematic exposures between the TP and HTD were individual and population bioequivalent (95% upper-confidence bound < 0, linearized bioequivalence criteria). This indicates that the HTD is a suitable surrogate for a wheelchair user in low-speed, low-impact wheelchair studies.

Miracle mineral solution Solution and how it Works

Admin October 17th, 2009

miracle mineral solution Solution and how it Works

The miracle mineral supplement, MMS, lies in the powerful molecule chlorine dioxide. Chlorine dioxide has a tremendous capacity to work in unison with the body?s defensive physicalmechanisms. Unlike antibiotics, it works with the body?s immune system. As you take more and more antibiotics, the body builds up a resistance to them and they become ineffective. Thepharmaceutical companies are constantly fighting this problem. They have to keep developing new drugs when the ones they manufactured formerly have become ineffectual. Drug resistanceis becoming a bigger and bigger problem.
