The effect of providing power mobility on body weight change

OBJECTIVE: To investigate whether the provision of power mobility would have an effect on body weight in adults who were first-time qualifiers for power mobility. DESIGN: This was a retrospective observational study of consecutive subjects, who served as their own controls. The medical scooters, handicap scooters records of 468 subjects who were approved for power mobility during a 17-mo period were reviewed. Three weight groups (12 mos before, at, and 12 mos after the power mobility evaluation) were evaluated with repeated-measures analysis of variance (ANOVA). The weight changes on subjects in different age groups (45-54, 55-64, 65-74, and >74), in geriatric vs. nongeriatric groups, and in different body mass index (BMI) groups were analyzed. RESULTS: Eighty-nine subjects met the inclusion criteria. They were obese (49.4%), and most of them were geriatric (64%). Congestive heart failure (30.34%) and chronic obstructive pulmonary disease (22.47%) were the two main presenting diagnoses. The repeated-measures ANOVA showed no significant weight change in the three studied weight groups. Similar results were seen in the age and BMI subgroups. CONCLUSIONS: There was no statistically significant weight change in adults who were first-time qualifiers and who used power mobility for 1 yr.

Hand-rim wheelchair propulsion capacity during rehabilitation of persons with spinal cord injury

This paper describes the course of power wheelchairs and handicap scooters propulsion capacity (WPC) during rehabilitation of persons with spinal cord injury (SCI) and its relationship with personal and injury characteristics. We investigated 132 subjects with SCI (37 with tetraplegia) at the start of active rehabilitation (t1), 3 months later (t2), and at the end of clinical rehabilitation (t3). WPC was measured as the maximal power output that can be achieved in a maximal wheelchair exercise test on a treadmill. Results were analyzed with the use of generalized estimating equations, with time of measurement, lesion level, motor completeness of the lesion, age, and gender as independent variables. Overall, WPC increased from 30.5 W at t1, to 39.5 W at t2, and 44.2 W at t3. Persons with paraplegia, persons with incomplete lesions, men, and younger persons had higher values for WPC compared with persons with tetraplegia, persons with complete lesions, women, and older persons. Rate of improvement was lower in older persons and women compared with younger persons and men. This paper identifies factors that affect the level (lesion level, completeness of the lesion, age, gender) and rate of improvement (age, gender) of WPC during rehabilitation. These findings should be considered when wheelchair capacity training is applied in SCI rehabilitation.

Evaluation of pushrim-activated power-assisted wheelchairs using ANSI/RESNA standards

OBJECTIVE: To determine and compare performance of pushrim-activated power-assisted wheelchairs (PAPAW) (iGLIDE, e-motion, Xtender) on national standards. DESIGN: Engineering performance and safety evaluation. SETTING: A Veteran Affairs and university-based research center. SPECIMENS: Nine PAPAWs. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Static, dynamic stability, brake effectiveness, maximum speed, acceleration, retardation, energy consumption, static, impact, and fatigue strength. RESULTS: There was no significant difference among the 3 models in forward stability. The iGLIDE was the most stable, whereas the e-motion was the least stable model in the rearward stability tests. All PAPAWs performed equally on the slopes of 3 degrees and 6 degrees in the forward and rearward directions. Braking distance was the highest for e-motion (5.64+/-0.28m) and the lowest (1.13+/-0.03m) for the iGLIDE in forward direction. The average equivalent cycles of all PAPAWs were 318,292+/-112,776.6 cycles (n=8) on the fatigue tests. All PAPAWs passed the impact and static strength tests. CONCLUSIONS: The standards of the American National Standards Institute and handicap scooters, power wheelchairs the Rehabilitation Engineering and Assistive Technology Society of North America could act as quality assurance tool for wheelchairs. The standards for wheelchairs were first approved in 1990; after 17 years, exceeding the minimum values in the standards would be a reasonable expectation
